
Health Services
Health services are provided according to Colorado Department of Education guidelines and as directed by the Liberty Common School Board of Directors. The school maintains health records which are kept in the student's cumulative and/or health folder. Each Liberty Common School campus has a health technician on site with Registered-Nurse supervision.

Vision and Hearing:
As required by CDE guidelines, the vision and hearing of students in kindergarten, first, second, third, fifth, seventh, and ninth grades, or of all students in comparable age groups referred for testing, are tested for deficiencies during the school year. The goal of these screenings is to identify children who have a need for further diagnostic testing and prevent delays in development.

All personal exemptions must be renewed annually and must be dated July 1 or later to be acceptable for the following school year. Further details and instructions can be found in the link.

Procedures for Medications:
- All medications—prescription and non-prescription—require forms to be completed
- Medications need to be unexpired and unopened, in their original packaging (as small as possible) or prescription container with the most current label. Medications must be turned in to the health office, with the appropriate medication authorization form, by the parent/guardian.
- If the parent or guardian is not available to drop off or pick up medication, contact the health office to make other arrangements. Do not send medications with students.
- High-school students may self-carry and self-administer any over-the-counter or prescription medications (excluding controlled substances) with the self-carry form.
- Students of any age may self-carry emergency medications such as asthma inhalers and Epi pens with the permission of the parent, doctor, and school nurse with the proper self-carry forms.

- If your child has a health need requiring support during the school day or if you have questions or concerns, please contact your school nurse Natalie Dybzinski at ndybzinski@
libertycommon.org. - If anything changes regarding your child’s health throughout the school year, please also reach out to your school nurse, health tech, and child’s teacher if applicable.
- If your child is participating in a sport or extracurricular activity, please be sure to include their coach or leader directly in any health information they should be apprised to.
