Letter from Health Services

Dear Parent(s)/Guardian(s),

This letter explains various health and medical details related to school. If you have further questions or concerns after reviewing this information, please reach out to the school nurse or the health technician at your student’s campus using the contact information listed below.

Immunizations and Exemptions:

The state of Colorado requires that all students entering a Colorado school must be up-to-date on all  immunizations.  The health-services page of our website links to a letter and guidelines from CDPHE listing the immunizations that are required for schools. 

• Please make sure your student’s immunizations are up-to-date so his/her record can be turned in during the annual update and contact your healthcare provider with any questions regarding missing immunizations.  You may bring the guidelines with you to your provider if you would like. 
• Non-medical exemptions require a doctor’s signature on the official form and/or completion of the state-required learning module annually.  Additional information on this can be found on the immunization letter from the CDPHE.  Children with an exemption may be kept out of school during a disease outbreak if recommended by the county.  
• Medical exemptions require a physician’s signature and need to be submitted only once. 
• Without proof of current immunizations, your child may not start school.  Please contact your child’s healthcare provider to obtain records if needed.  If there are immunizations that are lacking, you will be advised as to what your child needs to be up-to-date, and these must be addressed in a timely manner. These can be turned in July 1 and onward of the current school year.

 

Health Concerns:

• Please provide details during registration and on the annual update about any health concerns your child has so we may be of assistance to your child at school.  This information will be included in a medical alert in their student health system so those who need to know are aware. 
• If your child has a health need that requires support during the school day or if you have questions or concerns, please contact your school nurse Natalie Dybzinski at ndybzinski@libertycommon.org before school begins. 
• If anything changes regarding your child’s health throughout the school year, please also reach out to your school nurse, health tech, and child’s teacher if applicable. 
• If your child is participating in a sport or extracurricular activity, please be sure to include their coach or leader directly in any health information they should be apprised to.

 

Role of The Health Office:

• Each school has a health technician who provides daily care for students that may include:
  • Administration of medications
  • Care for minor injuries and wounds
  • Illness monitoring
  • Communicating with parents/guardians, school staff, and school nurse if necessary.  
• Our school nurse delegates to the health technicians and supervises them according to the Colorado Nurse Practice Act.  Neither the school nurse nor health technicians are licensed to diagnose, so please utilize your family healthcare provider for continuation of care. 
• Please review the list of requirements as to when you should keep your child home from school for health-related issues on the “How Sick is Too Sick?” Keep Kids Home policy document.  We also follow the Colorado Infectious Diseases Guidelines.

 

Medication at School: 

All forms are available on the Liberty Common Health Services page.

• The “Authorization for Medication Administration” form must be completed annually with the required signatures for each medication. “Health Care Action Plans” must also be updated annually.
• Medication must be provided and brought to school by the parent, in the original prescription bottle or over-the-counter container, and must not be expired.  
• If medication changes or medication is discontinued during the school year, please have the child’s doctor fax the new or correct orders, or bring in the form to the front office.  
• Asthma inhalers or medication to treat severe allergic reactions may be carried by the student as long as this is agreed upon by the child’s healthcare provider, parent, and school nurse, and a health care plan is in effect.
• Only high school students may carry any medications for which we have a self-carry medication form on file and which we approve.  This authorization to carry can be revoked at any time should the student not demonstrate the proper responsibility for this privilege.   

 

Phone Numbers: 

It is important the school has your home and work numbers on files, as well as emergency contact(s) and phone number(s).  Should your child become ill or hurt while at school, we must be able to reach you and someone must be able to pick your child up quickly if needed.  If changes occur during the school year, please notify the front office of your student’s school.
